A wellbeing check is a quick read on how much energy people have left: whether they recover between working days, keep a balance between work and private life, and know where to turn for support.
9 questions, about 2 minutes, and one open question on what takes the most energy right now. It is deliberately not a health questionnaire: for a deeper, individual look at burnout risk, point people to the free Burnout Compass.

Original Y2Y questions. Practical survey questions written by Y2Y for organisations. They are not a psychometric or clinical test.
Important
This check asks about experiences at work, not about health. It is not a diagnostic or clinical tool, and its results must not be used to judge an individual's health.
Anonymity
Answers are anonymous. Nobody, including the company, sees an individual response: results appear only for groups, and only once at least 5 people have answered. A breakdown (for example by department or site) appears only for groups that reach 5 answers on their own.
Run it with your team
Your Assessor sends the survey as an anonymous campaign: invitation links by e-mail, a live results dashboard with breakdowns, CSV export. Ready-made questionnaires come with a company account; tell us your headcount and we send access and a quote.
Related questionnaires
- Psychosocial risk assessment questionnaire
Input for your psychosocial risk assessment by site or unit, followed by an action plan; it supports the assessment, it does not replace it.
- Health and safety climate survey
For production, logistics and site work: how seriously safety is taken day to day (psychological safety is a separate questionnaire).
- Onboarding survey (30 and 90 days)
Ask new joiners after 30 or 90 days; keep it open for several months so that 5 answers come together.
